Export Control Laws. Information, technology, and other materials are subject to federal export control statutes and regulations, which have been set in place for such purposes as national security, antiterrorism activity, trade sanctions, and more. These statutes and regulations restrict the "export" of these concepts and materials to certain non-U.S. citizens without obtaining an appropriate export license, unless they are a U.S. Permanent Resident or a Protected Individual under U.S. law. DXC has implemented a badge process to identify employees who cannot have access to certain export-controlled information until any necessary licenses are obtained.
Verification of citizenship, which establishes one’s nation of citizenship, is not the same process as completion of Form I-9, which establishes eligibility to work in the U.S.
To comply with federal export control statutes and regulations, DXC must identify and verify the country of citizenship for all employees assigned within the U.S.
U.S. employees on assignment overseas who have not had their citizenship verified by DXC are required to complete the verification process upon return to the U.S.

- If you are a U.S. citizen or national*, provide a Certificate of U.S. Citizenship (Form N-560 or N-561), or your U.S. passport (expired or unexpired), or the original or a certified copy of your U.S. birth certificate. If you are a naturalized U.S. citizen, provide your Certificate of Naturalization or U.S. passport (expired or unexpired).
- If you are a U.S. Legal Permanent Resident (AKA "Resident Alien"), provide your Form I-151 or I-551 Permanent Resident Card (AKA "Green Card"), or Alien Registration Receipt Card with photograph, or U.S. passport (expired or unexpired).
- If you are a Protected Individual (e.g., refugee, asylee, etc.), provide the documents from the U.S. Citizenship and Immigration Services Department that substantiate your claimed status or U.S. passport (expired or unexpired).
- If you are not eligible to claim status in 1, 2, or 3 above, you must provide a passport (expired or unexpired) from the country for which you are claiming citizenship.
- If you claim more than one citizenship, provide documentation supporting citizenship in each country claimed.
* "U.S. citizen" includes citizens of the 50 states, the District of Columbia, Puerto Rico, the U.S. Virgin Islands, Guam, and the Northern Mariana Islands. All U.S. citizens are considered to be U.S. nationals. However, not all nationals are U.S. citizens: natives of American Samoa and Swain's Island are not U.S. citizens but are nationals.
- Complete all areas of Section 1 – "Employee Information". If the name on your document differs from your name in DXC records, attach a dated and signed statement to your form which explains why your name appears differently. If you require a translator or assistance in completing this form, ask your translator or individual providing assistance to complete the Preparer and/or Translator Certification section.
- Take your form and proof of citizenship documentation to an authorized document reviewer (DXC HR or Security representative, your manager or supervisor or, by exception, a notary public approved by DXC) to review and verify. Be sure to RETRIEVE YOUR DOCUMENTATION once the document reviewer has completed his/her section of the online form.
If You Cannot Locate Your Citizenship Documents, Follow this Process
- First, complete the "Employee Information" section of the Citizenship Verification form and submit it to your document reviewer.
- Next, immediately contact the appropriate issuing authority (see below) to obtain a new document or to replace your missing document. Be sure to get a RECEIPT that shows you have requested the document.
- Provide the receipt to your document reviewer along with the form. You are required to provide the official document to your document reviewer within 90 of applying for the new document. Your reviewer should keep the form and the receipt until you provide the documentation.
Where to Obtain New or Replacement Documents

Exceptions to this Process
- An employee currently on OVERSEAS ASSIGNMENT does not need to complete this process until s/he returns to work in the U.S.

What should I do if I can't find my required citizenship documents?

Who is my document reviewer?
For most employees, the document reviewer will be their manager. If you are located offsite, your manager may assign the responsibility to an alternate document reviewer, such as another manager or HR staff member, located closer to you.
How do I locate my document reviewer?
If your manager has assigned the responsibility of verifying your documentation to an alternate reviewer, your manager should be able to assist you in locating that person. If you cannot contact your manager, please ask your business unit HR department for assistance.
What should I do if I'm located too far from my assigned document reviewer?
Contact your manager, who will work with your business unit HR department to locate a document reviewer closer to you.
I hold citizenship in more than one country. What documentation do I have to present?
You must present proof of citizenship for ALL nations of which you are a citizen. (See Acceptable Documentation above on this page.)
Who must comply with DXC's citizenship verification process?
All U.S.-based DXC employees must present proof of citizenship. (This includes full-time, part-time, and casual employees.) DXC employees on assignment overseas and employees on a leave of absence must present proof of citizenship upon return to work at a DXC location based in the United States.
The last name on my birth certificate does not match my current last name. What should I do?
If the name on your birth certificate (or other document) does not match your name as reflected in DXC's HR/Payroll system records, enter your Other Name (located directly on the first line of Section 1) on the form, and attach an explanation to the form as to why your name does not match in both places. Please sign and date your attached explanation.
EXAMPLE: DXC employee Jane Doe was born Jane Smith. In the "Other Name" section, she enters "Jane Smith." Jane will indicate in her explanation, "I assumed my husband's last name when we married," and sign and date her explanation.
Who is responsible for returning the completed Citizenship Verification form to Corporate Shared Services?
Once the Citizenship Verification is complete, either the employee or the reviewer must send the form to Corporate Shared Services - U.S., Attention: Citizenship Verification Department.
