A platform approach to creating a sustainable digital ecosystem for patient-driven care
In a world where patient expectations are growing, with increased digital literacy, we need new models of care and technology to fast-track its delivery. Considering the challenges of fragmented data and the cost of replacing existing applications, the only viable solution is to build a sustainable, scalable digital ecosystem that will provide a single view of the patient, real-time analytics, a secure way to share data and provide a platform for innovation. To truly empower patients to better manage their health, we need access to patient data across the healthcare system and the ability to derive contextual insights to allow clinicians and patients to make informed decisions about their health. This presentation offers a pragmatic perspective to help shape your digital journey.

Patient-Drive Care turns traditional care delivery models upside down
Engaging with patients has become a priority for healthcare and life sciences organisations, and many are investing in tools that create the types of experiences patients enjoy in other parts of their lives. This shift is so significant that 72% of providers are implementing patient-engagement solutions.
Patient-driven solutions can bridge the chasm between patients and healthcare providers and deliver better and more efficient health outcomes. The key is generating insights that positively effect clinical decisions and help drive better outcomes, including shortening hospital stays, reducing patient admissions and enhancing care quality.
